Transitional Housing Programs
The following programs offer transitional housing. Stays range from 90 days to 2 years.
James F. Boccardo Regional Reception Center
Location: 2011 Little Orchard Street, San Jose,

Serves: Single adults, homeless veterans and families
with children
The Boccardo Reception Center offers 90-day transitional housing with
individualized case management, life skills workshops, basic needs
assistance, tutoring for children, and other supportive services.
This site also offers emergency shelter.
John H. Boccardo Family Living Center
Location: 13545 Monterey Road, San Martin, 408-686-1300
Serves: Families with children
The Boccardo Family Living Center provides support to help families
transition from homelessness to permanent housing. Services include
individualized case management, life skills education, ESL classes,
children’s education and recreation programs, and more. This
facility also offers emergency shelter and migrant farm worker housing.
Our House Transitional Living Program
Location: 282 E. Younger Street, San Jose, 408-292-6915
Serves: Emancipated youth and young adults ages 16
to 21
The Our House Transitional Living Program helps youth and young adults
learn how to live independently through on site case management and
life skills instruction. Participants must be working or in school,
and monthly mandatory savings payments are put into a savings account
for use upon graduation from the program.
Transitional Housing Program Plus (THP+)
Location: Scattered sites
Serves: Former Foster Youth ages 18 to 24
The THP+ program offers up to 24 months of supportive services, case
management and financial support for rent and groceries while youth
work to achieve educational and employment goals. Youth are referred
by the County of Santa Clara Independent Living Program.
Sobrato Family Living Center
Location: 1509 Agnew Road, Santa Clara, 408-539-2155
Serves: Families with children
This transitional housing provides individualized case management,
life skills workshops, basic needs assistance, tutoring for children,
clinical counseling, and other supportive services. Long term supportive
housing for families with disabilities is also offered at this site.
