Licensed Clinician

Position: Licensed Clinician
Location: Boccardo Reception Center (BRC)
Supervisor: Program Manager

Duties and responsibilities:

  • Conduct regular case management sessions to monitor client’s progress toward goals, advise, make appropriate referrals to other service providers, and provide support to help them achieve their goals.
  • Provide ongoing assessments to review medical, mental health and addiction needs.
  • Develop Individual Care Plans for 15 mentally ill homeless participants to help them obtain the skills and income needed to move into stable housing.
  • Provide referrals to assist program participants in their transition to permanent housing.
  • Work with the County Department of Mental Health system on the delivery of appropriate services and issuance of medications.
  • Collaborate with the County Department of Mental Health to review referrals and intake appropriate clients.
  • Facilitate regular meetings and case reviews with the County Department of Mental Health.
  • Provide system of care linkage and transfer so clients can receive Outpatient Services and/or services from a primary health care provider as consistent with individual choice and need.
  • Attend ongoing mental health trainings, including but not limited to: Basic Mental Health 101, Suicide Assessments, Safety Precautions and HIPAA regulations.
  • Provide crisis intervention when needed.
  • Maintain thorough, accurate records of case management activities with every program participant.
  • Participate fully as a member of the BRC team and attend weekly staff meetings and trainings as required.
  • Possess knowledge, adherence and enforcement of EHC LifeBuilders’ policies and procedures.
  • Provide complete clinical assessment for up to 40 homeless families annually.
  • Assist program staff in providing drug and alcohol prevention services in formal and informal group settings as well as on outreach.
  • Completion of all appropriate paperwork and tracking of services.
  • Develop and instruct prevention workshops.


Qualifications:

  • Licensed Clinical Social Worker OR MFT (License is required for this position).
  • Extensive knowledge of mental health system of care, community resources and client needs.
  • Minimum of three years experience working with the mentally ill.
  • Sensitivity to the needs of homeless individuals.
  • Possess a current drivers license, reliable transportation and vehicle insurance.
  • Knowledge and skills in the principles of psychosocial rehabilitation and recovery processes.
  • Strong communication (both written and oral) and organizational skills.
  • Ability to work with people of diverse social backgrounds and professional rank.
  • Experience in active listening, crisis intervention, and mediation skills.
  • Good collaboration skills and the ability to work independently with limited supervision.
  • Bilingual in Spanish preferred.
